Form’H@D is a solution digitizing at source all information entered into the paper file available at the patient's bedside.Kayentis is behind a "unique electronic and paper solution for hospitalization at home" called form'H@D. It combines the best of the physical and digital worlds. form'H@D offers a simple and intuitive tool enabling all professionals involved in the care pathway to connect to each other using pen and paper. Form'H@D collects, manages and secures the data of a patient maintained or hospitalized at home. This information is entered into the patient’s file using the digital pen and transmitted, either through a PC linked to the internet, or a telephone in Bluetooth mode. The information written by the care staff is stored in the pen and is then transferred in a secured mode to a web server for validation before automatically transmitting it to the HH's information system.
Form'H@D allows an HH or any other home care set-up to operate: all the procedures recorded at the patient's home are monitored and archived in the hospital's information system. The technology used by the digital pen and paper is fully adapted to this type of situation. It optimizes not only the gathering of the data since it is an intuitive and general tool, but also the quality of these data. It is currently the best method of excluding nobody and thereby speeds up the adoption of new technologies applied to the health sector.
